Longitudinal prospective study: Analyse the extend of medication error

Medication error are one of the most frequently occurring types adverse events in hospitalized patients and potentially more harmful children than adults. A prospective study was carried out to identify incidence medication selected hospital, Indore. 16355 records patients, admitted Indore were evaluated. Prospective aimed evaluate compliance, completion & accuracy audit checklist case files accredited hospital City. Data collected by examining patient’s record, their feedback observation administration practice concerning nurses through prescription, transcription as per NABH norms. reveals Prescription faults errors (62.3%) common comparison (25.9%), (11.8%) preservation drugs (0%) related errors. all parameters ranged above 60% that indicates corrective steps strategies has be adopted for reducing risk wrong processing medication. analysed with descriptive statistics is frequency percentage.
